Host Family Reports (Summer)

Note:

All host families should complete the Week 1 Report at the end of the first week of hosting a child regardless of the child's prior stay in other households.

Note:

All host families should complete the Week 3 Report.

Reminder:  Please have your host child's weight and height determined by the end of the program. We will request this information in the Week 5 Report.

Note:

Families in 4-week programs, or families whose child has reached the penultimate week of camp, are requested to complete both Week 4 and Week 5 reports in the fourth week.

Note:

All host families need to complete the Week 5 Report regardless of the length of the stay.
