Summer Miracles Host Families |
| Week 1 Report - ALL HOST FAMILIES should complete the Week 1 Report at the end of the first week of hosting a child regardless of the child's prior stay in other households. |
| Week 2 Report |
| Week 3 Report - ALL HOST FAMILIES should complete the Week 3 Report. |
| REMINDER: Please have your host child's weight and height determined by the end of the program. We will request this information in the Week 5 Report. |
| Week 4 Report - Families in 4-week programs, or families whose child has reached the penultimate week of camp, are requested to complete both Week 4 and Week 5 reports in the fourth week. |
| Week 5 Report - ALL HOST families need to complete the Week 5 Report regardless of the length of the stay. |
